Normally, the uterus rests over the urinary bladder in 60% of women. But in 40 % of women, the Uterus tends to bend backward, pointing away from the bladder and more toward the rectum. This is called Retroverted Uterus
Different Positions of the uterus

The Cocleate uterus is associated with severe abdominal cramps during periods, and the retroverted uterus can give severe backache during periods and during pregnancy as well.
The Retroverted Uterus and Pregnancy-
The extremely retroverted or the retroverted cochleate uterus can be associated with severe cramps during the first 3-4 months of pregnancy. It can also lead to retention of urine (inability to pass urine) due to pressure on the urinary canal or urethra. In severe cases, the growing fetus and the upper part of the uterus can be trapped in the pelvis- a retroverted incarcerated uterus. This can be extremely painful and may lead to a miscarriage.
